I was surprised to not see any red eyes lora for realistic, as realistic models are natively not able to make people with red eyes.

Focus on the eyes, make close up of he eyes
Make other things in the picture red, if you want clothes of another color, you can prompt it, it will most of the times work to make both the eyes red and the clothes another color ! :)
A weight of 0.8 to 1 is fine. You may lower it to 0.5 for a more natural effect.
